Toggle Comment works across keyboard layouts and Shift+Insert pastes on Windows and Linux

Toggle Comment does not respond to the natural comment-marker shortcut on some non-US keyboard layouts because its punctuation key binding is layout-dependent. On Windows and Linux, Shift+Insert is also not recognised as an alternate ordinary Paste shortcut. Toggle Comment should recognise the # symbol across keyboard layouts while retaining its existing binding, and Shift+Insert should perform the same context-aware paste as Ctrl+V.
