Insert menu shows duplicate Enumeration commands with different destinations

The Insert menu contains two commands both labelled "Enumeration", but they create enumerations in different places. When a module is open, choosing the project-level entry can create an enumeration in the project instead of inside the module being edited.

There should be one Enumeration entry in the Insert menu. It should add an enumeration inside the active class or module when that context supports it, and create a standalone enumeration when working in a project, Shared Code, or source folder. The toolbar Add menu and keyboard shortcut should follow the same context rules.
