AI Assistant thinking and tool calls now appear chronologically

AI Assistant responses that use tools should display each thinking phase and tool call in the order they occurred. Currently, later thinking is appended to the first thinking block, so the visible transcript groups all thinking before all tool calls and misrepresents the response timeline.
