AI Assistant token usage is now clear and complete

AI Assistant token reporting now clearly distinguishes generated output from total input and output usage, and includes usage from every internal model round. Previously, the response footer and conversation header showed apparently conflicting token counts, while tool-assisted responses could omit usage from earlier rounds.
