AI diagnostics now capture the complete conversation

AI assistant diagnostics should export the complete current conversation, preserving every user send and every provider/tool/repair round until the conversation is cleared. Reports should make it obvious which outgoing messages are prior context, the current user request, current-run state, or ephemeral repair context.
